Client Info & Questionnaire – Deducting Mortgage Loan Interest Under the New Tax Law

New Clients

For new clients, we have provided a few PDF forms that will make your tax preparation quicker and easier. Please complete the following forms that are at the top of this page:

  1. A blank 2018 organizer: Choose either the long or short organizer from above based on your tax situation and complete it to the best of your ability. If you have a very basic return, please use the short organizer. If you have a more complex tax return, please use the long organizer.
  2. An individual engagement letter: Please sign this. It gives us authorization to prepare your return
  3. Affordable Care Act Information Intake Form: Please read this and mark the option that applies to you and your family.
  4. Tax Return Drop Off Checklist: If you are going to drop off your tax documents, please print this form and fill it out before you come to save you time.
  • Please also provide us with a copy of your 2017 tax return and your source documents for the 2018 tax year (W-2s, 1099s, K-1s, etc.).

Once you have everything completed and ready, please contact our office at 480-926-0672 to get set up with our SmartVault portal system so you can upload all of your documents there. SmartVault is just as easy as using email, but it is significantly more secure than using email to get your documents to us.


Existing Clients

 If you are already a client, please DO NOT fill out the organizer above. You should have  a Custom Client organizer in your SmartVault portal vault here : SmartVault Portal. If you do not know how to access your SmartVault vault, please contact our office and we will help you get your vault set up.  However, please DO fill out both the Tax Return Drop Off Checklist and Affordable Care Act Information Intake Form, as this will help us accurately expedite the process of preparing your return.
